I ran through this drill last year and it completely changed my result off the tee, arrow straight to a minor draw. I show it to everyone I can who has a natural fade to a slice miss...I used Hank Haney's version, but it is essentially the same thing. ▶ this video

Your hands get too deep in the backswing. Stand up straighter and closer to the ball. This will likely make your swing less flat. Hank Haney has a good drill and feeling for this. He explains it here : ▶ this video I think you’re very close to having a nice swing

IMO, your biggest issue is the overall swing motion. Your backswing is way inside, which causes you to compensate on the way down with an over the top motion, or out to in path. Check out Hank Haney’s drill to get the opposite feeling of a clockwise hand motion during the swing.▶ this video
