There's much to fix in your swing. If possible, get lessons. You are reverse pivoting and extending early. You're also overswinging a bit which promotes these problems and separates your right elbow too much. Reverse pivoting means you shift your weight to front foot in backswing, and move it to trail foot in downswing.
