You are the 3rd outside backswing in a row that I've looked at. I can't see your stance but ultimately, it's your backswing. Everything else is just an adjustment to correct. It causes the slightly early extension and it causes the outside followthrough that looks like you're shoveling something over your left shoulder. Double check stance and grip.
