Another way to think about this is that your shoulders should be at an angle that generally points at the golf ball at the top of the backswing (if you did that, it should result in your hands getting some height from your shoulders). Pause the video when you have the club at the top. Your shoulders are almost completely horizontal. It's near impossible to make a proper swing from that position. This is a good drill …

Your head moves around like that because you aren't adding in some left side bend in the backswing. It would help keep your head "still". search youtube for left side bend. example ▶ this video it'll also help get your club above your head more than behind your body Add some more wrist cock earlier as well.
